Certified Births, Deaths, Marriages and Single Status Document
1、The original document issued by Registration of Births, Deaths, Marriages Office should be provided.(Full version, do not laminate)
2、Include a photocopy of your passport, or photocopy of the parents’ passports in the case of Birth Certificate (Photocopy of ID must be certified by Justice of Peace).
3、Download and complete APPLICATION FORM FOR AUTHENTICATION.
4、Fees: $22 Dollars for each document. (Expedited service suspended)Expedited services for urgent application can be requested by paying an additional 50% of Authentication fees, which is AUD11. Payment must be made in cash (not for mailing application) bank cheque or money order and payable to the "Taipei Economic and Cultural Office in Australia". *Personal cheque will not be accepted.
5、Walk-in applicants : (by appointment only)
Postal - applicants : Prepaid envelope for return of documents。Applications lodged by post or courier should also include self-addressed postage prepaid envelope (Express Post Platinum, Express Post and Registered Post B4(353mm×250mm)only) or courier satchel for the return of documents.
*If a foreign spouse is unable to return to the R.O.C. with his/her R.O.C. spouse, the foreign spouse must submit his/ her Chinese name declaration certified by a R.O.C. foreign mission. (For Marriages certificate), Chinese Name Declaration must be signed in person in front of our officer at office (by appointment only). Postal applicants are required to have the Declaration notarized by WA/ACT Notary Public prior to submitting the document to our office.
