When you purchase a Bank draft or Bank Cheque, please make sure the cheque contains the Magnetic Ink Character Recognition (MICR) code.

The valid original passport and one photocopy of the applicant
The original passport will be returned to the applicant after the process.
If original passport cannot be provided, a verified true copy of the passport must be obtained from a passport office or Service Canada (for Canadian passports), or the government authorities (e.g., the embassy, commissioner, or consulates) for passports of countries other than Canada and Taiwan.
A business card or contact information (including email address) must be provided so that we can contact the physician to verify the certificate.
This form is also for application for Youth Mobility Program (Working Holiday).

- Greater Ottawa Region, Quebec: Please provide Regional return envelope; Newfoundland and Labrador: Please provide National return envelope.
*If the Health Certificate is for visa application, the authenticated Health Certificate will be included in your visa application package; thus you do not have to provide an extra return envelope for this application.
