A completed passport application form
A certified birth certificate and Chinese translation (must be authenticated by Ministry of Justice and Human Rights of the Republic of Indonesia, Ministry of Foreign Affairs of the Republic of Indonesia, and TETO ).
The parents' marriage certificate and a Household Registration Certificate with the parents' marriage registration within three months.
Copies of personal information page of BOTH parents' passports, and one original passport from Father OR Mother (the original passport will be returned after viewing)
Two identical passport photos of the applicant.
Agreement document on the child's Chinese surname and signed by both parents.
