Required Documents
- Certificate of Entry Application Form (fill out in Chinese)
- Signature: The "Applicant's Signature" field must not be left blank.
- Applicants aged 7 and above must sign personally.
- For children under 7 who cannot write, the parent may write "Full Name of the Person + (Father’s Representative) or (Mother’s Representative)."
- Contact Information: Ensure that the phone number and email address fields on the application form are filled out clearly for contact purposes.
- Photographs: Provide 2 color passport-sized photographs with a white background taken within the last 6 months. The distance from the chin to the top of the head (including hair) should be between 3.2 to 3.6 cm. Refer to the photo specifications for details.
- Submission of Republic of China (Taiwan) Passport:
- Expired Passport: Submit the original and a copy of the expired Republic of China (Taiwan) passport.
- Lost but Not Expired Passport: Submit the police report issued by the New Zealand police and a copy of ID or household registration transcript or other documents proving Republic of China (Taiwan) nationality.
- The police report must provide a detailed account of the passport loss.
- If the local police authority has not issued or does not issue a police report, please submit a statement explaining the lost passport, along with the 105 online reporting system.
- Lost and Expired Passport: Submit a copy of ID, household registration transcript, or other documents proving Republic of China (Taiwan) nationality.
- New Zealand Identification Documents (Original & Copy): This includes documents such as New Zealand passports, PR, visas, or citizenship certificates.
- Passport Copies: The Republic of China (Taiwan) and New Zealand passports can be copied on the same page.
- For Mailing: you may choose to mail either the original or a copy.
- Submitting Copies: Ensure that the original NewZealand passport is signed. For colored copies, including the signature page, write "Certified True Copy" and sign and date the copy. The same applies to other residency documents. Failure to sign and date will delay processing or collection times.
- Mailing Instructions: Mark the envelope with "Passport Application Team.
- Travel Documentation: Provide proof of a direct flight ticket.
- For Applicants Under 18:
- Attach the guardian's valid identification document.
- Accepted documents for guardians include Republic of China (Taiwan) ID card, driver’s license, international driver’s license, passport, or household registration transcript.
- Also, provide proof of guardianship, such as the applicant's birth certificate or household registration transcript.
- For children of divorced parents, a custody consent form or an original and a copy of the court judgment must be submitted. These documents must be certified as true by a Notary Public within our jurisdiction, and then verified by our office.
- For applicants who are born out of wedlock (where the parents are not married), born before the parents’ marriage, or where the marriage has lasted less than 181 days, please see Certificate of no impediment to marriage or Divorce Decree.
- Parents should come in person to sign the consent form at our office.
If the Parents Are in Taiwan |
Parents should bring their ID cards and household registration certificates to the Ministry of Foreign Affairs' Bureau of Consular Affairs or to any of its local offices to complete the consent form. |
FEES
- NZD 9. Only Eftpos or cash payments are accepted.
- Application may be couriered: Please make sure you send us all required documents, our office will get in touch with you via email to provide payment details as soon as your application is received. If you are paying via bank transfer, please ensure to include the case reference number in the transfer details.
DURATION
- Approximately one week (excluding holidays, postal time, and situations requiring additional documents)
- To collect your documents, you must present the receipt. If you have someone else pick up the documents for you, they must show the receipt and their identification.
- If you need the documents mailed to a New Zealand address, please prepare a self-addressed postage-paid envelope with tracking, and include the recipient's details. Our office is not responsible for delays or losses once the documents are sent.
