Service Areas: Taipei Economic and Cultural Office in Los Angeles (TECO-LA) is only authorized to authenticate documents that have been issued within our service areas, covering Southern California ( Los Angeles County, San Diego County, Orange County, Riverside County, San Bernardino County, Kern County, Ventura County, Santa Barbara County, San Luis Obispo County and Imperial County), Arizona and New Mexico.
Requirements for Authentication Application of Death Certificate
A completed Application Form for Authentication. Please fill the deceased 's information on the "applicant" part of the form; the individual who submits the application, please fill the "agent" part .
The deseased passport or photo ID. If the deseased was a R.O.C.(Taiwan) citizen, please provide his/her R.O.C.(Taiwan) passport for cancellation.
Applicant's VALID passport or photo ID card
The application can be submitted by the informant who's name shown on the death certificate. If submitted by the family member of the deaseased, the relation certificate such as marriage certificate, birth certficat, or household registration is required as well.
If apply by mail or by an agent, a photocopy of the passport or ID notarized by a notary public can be accepted. If it's a R.O.C.(Taiwan) passport, its photocopy can be accepted without notarization.
The document intends to be authenticated:
The original or the certified copy of the birth certificate which was issued by the County/or the State. Please apply a complete form (long form), not an extract or a short form.
Please make sure that the deceased' s (1) English name (2) date of birth (3) place of birth showing on the death certificate are all the same as his/her passport information
If apply by an agent, (1)a notarized Letter of Authorization prepared by the applicant and (2)the agent's photo I.D. are required.
Authentication Fee: $15 for each
