Passport renewal:
If you possess a Taiwan passport valid for less than 6 months or is not a latest version, you are encouraged to renew your passport at the Taipei Economic and Cultural Office in Los Angeles (TECO-LA) or any other foreign missions of the Republic of China (Taiwan). Requirements and Procedure for general cases are listed below for your attention and preparation.
- A duly completed passport application form (pdf) (English_Sample)
- An original and most recent Taiwan passport
- Two identical passport photos (1.77x1.38 inches, color) (Guidelines for photo)
- A duly completed Exit and Entry permit application form (pdf) ( * apply to Taiwanese nationals born overseas/without a Taiwanese ID number upon application)
- If the applicant has a Taiwanese ID issued after December 21, 2005, the original Taiwan ID or the photocopies of two sides of the original Taiwan ID is required.
- Applicant's US passport, green card, resident visa or other photo ID.
- Other relevant certificates that may be required by TECO-LA for additional verification.
- Application fee: $45 (valid for 10-year)/ $31 (5-year)/ $10 (1-year).
First-time passport application(must apply in person)
- For applicants born to either a Taiwanese father or mother overseas and applies for his/her Taiwan passport for the first time, applications must be filed in person (*If the applicant is under 18 years old, one of the parents or guardian has to accompany the applicant for passport application) at TECO-LA or any other foreign missions of the Republic of China (Taiwan).
- However, to verify applicants' entitlement to Taiwan nationality, he/she must produce vital supporting documents, such as parents' marriage certificate and minor's birth certificate,and both duly authenticated beforehand by a designated foreign mission of Taiwan before submission.
- TECO-LA's designated service areas are Southern California, Arizona and New Mexico, whereas TECO-San Francisco, for example, covers Northern California, Utah and Nevada. *Applicants are highly advised to apply for "document authentication' and "first-time passport" at the same TECO office based on the principle of service area (where certificates were issued), rather than approximity (where applicants live).
Requirements:
- Apply in person
-If the applicant is a minor under the age of 18, the parents must come with the minor to apply in person. - Fill in the ROC(Taiwan) passport application form (English Sample) and sign on the back page
-If the applicant is a minor under the age of 18, the parents have to sign □ "Agreement on Minor’s Chinese Surname Named" to identify the applicant’s surname; if one parent is not a ROC(Taiwan) citizen and the parents agree that the applicant to bear the surname of the foreign father/mother, the foreign parent has to sign □"Chinese Name Declaration Form" to confirm his/her Chinese surname. - Two 2-inch photos taken within 6 months -must comply with the guidelines of the Bureau of Consular Affairs of the Ministry of Foreign Affairs
- Applicant’s valid U.S. passport
-Newborns who have not applied for a U.S. passport are exempted from the requirement. - Parents’ valid ROC(Taiwan) passport or foreign passport(if one parent is not a ROC(Taiwan) citizen)
-if the applicant obtaining the nationality of the Republic of China from the mother, the applicant must be born after February 10, 1980 - The applicant’s birth certificate authenticated by ROC overseas mission
- Please check (1) English name (2) date of birth (3) place of birth of the child and the parents showing on the birth certificate are all consistent with their passport information; if not, a name change document or other documents such as naturalization certificate, marriage certificate, old passports, driver's licenses, social security cards etc. must be provided to prove that they are the same persons.
- If the birth certificate was issued within our jurisdiction (Southern California, Arizona, and New Mexico), please apply for authentication at the same time. (please fill out the document authentication application form and pay the fee of US$15 for each) - Marriage certificate of the applicant’s parents -A marriage certificate or a certified copy of marriage record (showing the date of marriage) issued by a Taiwan Household Registration Office; or -If the parents got married abroad, the foreign marriage certificate must be authenticated by a ROC(Taiwan) overseas mission in charge. Please check (1) English name (2) date of birth (3) place of birth of bride and groom showing on the marriage certificate are all consistent with their passport information; if not, a name change document or other documents such as naturalization certificate, old passports, driver's licenses, social security cards etc. must be provided to prove that they are the same persons. And if the marriage certificate was issued within our jurisdiction (Southern California, Arizona, and New Mexico), please apply for authentication at the same time. (please fill out the document authentication application form and pay the fee of US$15 for each)
- Other documents if required (not limited to the following situations)
-If the parents of the minor applicant are divorced, and the parent with custody must provide □ authenticated custody certificate or a certified copy of a household registration record showing the custody of the minor.
-If the applicant born out of wedlock, □ the mother’s single certificate is required; and if the mother is not a ROC(Taiwan) citizen, the ROC(Taiwan) father must proceed the acknowledgment registration at the Taiwan Household Registration Office to legalize the relationship of the father and the child, and □ a certified copy of the acknowledgment registration record issued from the Household Registration Office is required - Passport fee, cash only -10-year passport US$45 -5-year passport(for applicant under the age of 14) US$31
Replacement of a lost or stolen Taiwan passport
(must apply in person)
If applicant's passport was lost or stolen, he/she must report it immediately to the local police authorities for a police record and contact TECO-LA or other foreign missions of the Republic of China (Taiwan) for a new passport. The following documents and fee are required for a new passport:
- A police report/certificate recording a Taiwan passport was loss/stolen, issued by the local police authorities ( *if the lost/stolen passport remains valid).
- A written Self-Description of how the passport was lost/stolen. (form download below)
- A duly completed passport application form(pdf)(example)
- Two identical passport photos (1.77x1.38 inches, color)(Guidelines for photo)
- A photo copy of Taiwan passport, national I.D. or certificate of Taiwan nationality (such as a Household Registration Record or a Driver's License
- If the applicant has a Taiwanese ID issued after December 21, 2005, the original Taiwan ID or the photocopies of two sides of the original Taiwan ID is required.
- Application fee: $45 (valid for 5 years)

Please attach a notarized copy of your photo ID as proof of your identity. However, a copy of Passport or ID Card is issued by the government of the Republic of China (Taiwan) will not required to be notarized if the applicant is a Taiwanese citizen. Prior to mailing documents to TECO-LA, he/she may contact Consular Division of TECO-LA staff at +1 213-389-1215 for guidance and further information. *Remember to attach a postage paid return envelop and put inside the mail envelope if you're unavailable to come and pick up the passport at our office. We only accept USPS returns, and $32 Priority Express Mail is recommended.
The Consular Division of TECO-LA is open Monday through Thursday (9:00 a.m. to 4:00 p.m.) and closed on U.S. Federal & California State holidays, Chinese Lunar New Year’s Day and October 10th.
