This declaration(取用中文姓名聲明書)is used for marriage registration at the Household Registration Offices in Taiwan when the foreign spouse will not be accompanying the R.O.C. (Taiwan) spouse to Taiwan.
Authentication requirements for Chinese Name Declaration in Queensland and Northern Territory:
Applications made in person:
Please make an appointment at tecobne.youcanbook.me or ring this office at 3828-1699.
On the day of your appointment, please bring:
- The applicant's valid passport original and 1 photocopy.
- The applicant must complete and sign the Application Form for Authentication. The original hardcopy is required.
- The applicant must complete the Chinese Name Declaration. Please leave the "SIGNATURE" field blank as our officer will need to witness your signature. The original hardcopy is required.
- Collection: Collect in person or provide a prepaid self-addressed Registered Post or Express Post envelope for the return of the authenticated court documents.
Applications lodged by post or courier should also include self-addressed postage prepaid envelope (Express Post Platinum, Express Post and Registered Post only) or courier satchel for the return of documents. - Authentication Fee

Applications by post:
The original Chinese Name Declaration must be notarised by a Queensland or Northern Territory Notary Public first. (Click here to find Notaries Public)
- The applicant's (and the representative's, if any) valid passport(s). (For postal applications, please enclose the photocopy of your passports instead.)
- Applicant (and representative, if any) must complete and sign the Application Form for Authentication. The original hardcopy is required.
- The applicant must complete and sign the Chinese Name Declaration before a QLD or NT Notary Public. The original Chinese Name Declaration has to be notarised by a Notary Public in QLD or NT.
- Collection: Collect in person or provide a prepaid self-addressed Registered Post or Express Post envelope for the return of the authenticated court documents.

- AU$22.00 per document for 5 business days standard application; or
- AU$33.00 per document for 1 business day urgent application.
Payment Method:
- Lodge in person: cash only.
- Postal applications: Money Order or Bank Cheque payable to the "Taipei Economic and Cultural Office, Brisbane". *Personal cheque will not be accepted.
