Attention: ALL ACREDITED HOSPITALS, CLINICS, MEDICAL CENTERS AND RECRUITMENT AGENCIES
With effect from 29 May 2015, all Taiwan bound resident visa applicants (including OFW) don’t need to undergo Serological Test for HIV for their health certificates. All accredited hospitals, clinics, medical centers are required to provide the Notice for HIV Screening and Treatment Costs to above applicants while issuing health certificates. 1.Items Required for Health Certificate(Form 2) (2 pages)： For OFW
2.Items Required for Health Certificate(Form B) (2 pages)：For general resident visa applicants
3.Notice for HIV Screening and Treatment Costs (2 pages): For all health certificate examinees.
