1. An online Visa Application Form (please access https://visawebapp.boca.gov.tw to fill up the Form and print them out)
2. Two passport-size photos in color taken within six months
3. Passport valid for at least six months and old passport showing previous travels

4. For the purpose of joining spouse, need to submit the following documents:
(1.) Birth Certificate issued by NSO
(2.) Marriage Contract issued by NSO and authenticated by Department of Foreign Affairs and TECO

(4.) Spouse’s passport (Photocopy only)
(5.) Alien Resident Certificate of spouse (Photocopy only)
(6.) Working Permit of spouse from Council of Labor Affairs (Photocopy only)
(7.) Medical Examination Clearance (Form B)from accredited Hospital & Clinics

5. For the purpose of joining parents, need to submit the following documents:
(1.) Birth Certificate issued by NSO and authenticated by Department of Foreign Affairs and TECO
(2.) Marriage Contract of the parents issued by NSO, authenticated by DFA and TECO
(3.) Parents passport (Photocopies only)
(4.) Alien Resident Certificate of parents’ (Photocopy only)
(5.) Working Permit of parents’ from Ministry of Labor of R.O.C.(Taiwan ) (Photocopy only)
(6.) Child above 6 years is required Medical Examination Clearance (Form B) from the accredited hospital and Clinics.
