Requirements for Authentication of Business Documents
1 | A filled up Application Form For Document Authentication (pdf). |
2 | Applicant’s original valid government-issued ID and 1 set photocopy |
3 | SEC Articles of Incorporation and By-Laws, original and 1 set photocopy. |
4 | SEC's Company's general information sheet (GIS) issued within 1 year from the date of application to this office, original and 1 set photocopy. |
5 | Business documents should be authenticated by the Department of Foreign Affair (DFA). |
6 | The Applicant should be one of the Board of Directors. If not, either one of the Board of Directors should execute an ORIGINAL authorization letter to their representative (The signature on the "Authorization Letter" shall match the one on the applicant's passport or government-issued ID). The representative’s original valid government-issued ID and 1 set photocopy shall also be submitted. |
(Document authentication application for Chinese translation): (2.) In the event that the applicant authorize a agent/representative for the business document authentication application, this office does NOT accept the agent/representative of applicant to sign the Chinese translation on behalf of the applicant. (3.) The document authentication application for Chinese translation shall be submitted together with the authentication application of the original copy. Please read: Notice: Authentication of Chinese Translation (4.) Chinese translation shall be prepared by the applicant. Translated format should be same as the English format. |
