Requirement for Death Certificate Authentication
1 | A filled up "Application Form For Document Authentication" |
2 | Applicant's valid passport original and one copy, if no passport, present any valid government-issued ID. |
3 | Original and one copy of the deceased passport or government-issued ID. |
4 | Original and one copy of the applicant's documents to prove the relationships with the deceased. |
5 | Death Certificate issued by the Philippine Statistics Authority (PSA) and authenticated by Department of Foreign Affairs (DFA) and 1 set photocopy. |
6 | In the case of claiming benefits or subsidies arising from the applicant's father, if the applicant born before marriage of the parents, or born less than 181 days from the date of the marriage of his/her parents, the Certificate of Single Status of the mother secured from the Philippine Statistics Authority (origianl with 1 photocopy) is required. |
7 | (Document authentication application for Chinese translation): (1.) Please confirm with the competent authority to see if the document authentication for Chinese translation is required before the application.(2.) The document authentication application for Chinese translation shall be submitted together with the authentication application of the original copy. Please read: Notice: Authentication of Chinese Translation (3.) Please also submit the ID documents or other proof documents that indicate the Chinese name and the English name of the applicant and parents (except for new born baby).
(4.) Chinese translation shall be prepared by the applicant. Translated format should be same as the English format. |
8 | (Application via agents or authorized representatives): (1.) Application can be lodged by an agent with an original "Authorization Letter" signed by applicant. The signature on the "Authorization Letter" shall match the one on the applicant's passport or government-issued ID.(2.) Note:
* Please make all photocopies on A4-size paper.
* In the event of the following circumstances, this office may request additional supporting documents or corrections to documents based on each case, and may notify the pickup time separately:
(1) If there are any doubts or concerns about the content of the document to be authenticated and need to be verified from competent authorities again;
(2) If the contents of the document to be authenticated do not conform to the supporting documents;
(3) If the contents of the document to be authenticated is blurry, unreadable, etc..
