2. Applicant’s valid and old passport and one photocopy of all pages used.
3. School Certificate of Enrollment should be authenticated by the Department of Education, Culture and Sports (DECS) or Commission on Higher Education(CHED), and the Department of Foreign Affairs (DFA). Original and 1 set photocopy.
4. If Mandarin Chinese translation authentication is necessary, please submit the translated documents (＊Translated format should be the same as the English format).
5. School Registration Card and Student ID Card. Original and 1 set photocopy.
6. Application can be lodged by an agent with an original "letter of appointment" signed by applicant (The signature on the "Authorization Letter" shall match the one on the applicant's passport or government-issued ID). The chosen representative must also present a valid government-issued ID during the application.
Note: Please make all photocopies on A4-size paper.
