If the applicant was born before Feb. 9, 1980, then the father has to be a Taiwan citizen in order to apply for a Taiwan passport.
Please come in-person to interview for the first time passport applicants, if the applicant is younger than 18 years old must accompanied by parents!
Processing time will take 8~10 weeks once we receive the application.
- Complete a passport application form, an Application Form for Entry and Exit Permit to Taiwan Area, Checklist for Applying for Entry Permit and Child’s Last Name Agreement (see next page). If the applicant is over the age of 18 then you can skip the last name agreement form.
- Your birth certificate and your parents’ marriage certificate all duly authenticated beforehand by a designated foreign mission of Taiwan before submission. TECO-SFO's designated service areas are Northern California, Utah and Nevada, the authenticated linkage is as below

(If the parents married in Taiwan, we need the original household registration transcript issued within three month.) - The ROC Taiwan passport of the parents. If you can’t provide the original passport, the copy must be authenticated by a TECO office. If one parent is not a ROC passport holder, complete a “Chinese name declaration form.” If the parent can’t sign in person at TECO office or the applicant applies by mail, the form must be notarized.
- If the applicant is younger than 18 years old, both parents must sign Section 11 of the application form below the applicant’s signature.
- Application fee is $45.00 for those 14 years old or older. The fee is 31.00 for children younger than 14 years old. Payment Method : Cash, cashier checks and Money Order (Make it payable to “TECO”). Credit card and personal check are not accepted.
- 3 passport photos taken within the previous six months. From the top of the head to the chin must be within 3.2 – 3.6 cm. It must show both ears, can’t show any teeth, and no hair band or eyeglasses.

*Parent’s ROC Taiwan passport, if one parent holds another country’s passport then notarize that country’s passport
*If the child does not have a US passport, then notarize both parents’ US passports or passport of another country if they do not have a US passport.
