- A Printout of visa application with signature (to apply online: https://visawebapp.boca.gov.tw/BOCA_EVISA/);
- The ORIGINAL passport with validity for more than six (6) months and a photo copy of the passport bio information page;
- Two photos (in color, passport-sized 45 mm in height and 35 mm in width) taken during the last six months;
- For non-US-citizens, a photo copy of US Permanent Resident Card or US visa;

- Visa application can be lodged by mail (attention: TECO NY, Consular Division; address: 1 E 42nd Street, NY 10017), drop-off at the ground floor lobby mailbox during office hours (9am-5pm, Monday through Friday), by appointment or walk-in-take-a-number (9am-1pm, Monday through Thursday).
- Visa application will not be processed unless all the requirements are met. You’ll be contacted by email if additional information or extra supporting document is needed.
- Slots for walk-in pickup are from 3 pm to 4 pm on cited day and following Tuesday and Friday, or by appointment or walk-in-take-a-number during our office hours. Please present the receipt and ID (of yourself or an agent on your behalf) for pick-up.
- A visitor visa without bearing the remark “NO EXTENSION”, enables you, with a valid reason, to apply for extension of stay for up to 180 days at a local immigration service center in Taiwan before expiration of the original duration of stay.
